This web annex provides a standardized short clinical facility assessment tool for monitoring the quality of viral load testing services and related data within HIV programmes. Developed as part of the WHO module for assessing and strengthening the quality of viral load testing data, it is intended for routine supervisory visits to clinical facilities and supports monitoring of critical steps across the viral load cascade in line with the UNAIDS 95–95–95 targets. The tool enables rapid assessment of service delivery processes, data quality and laboratory–clinic interfaces, and supports the development of facility-specific quality improvement plans. It is designed as a streamlined instrument for routine use, complementing the more comprehensive assessment tool presented in the accompanying implementation package. The annex includes standardized indicators, data collection methods and recording forms covering viral load testing practices, patient records, laboratory request forms, sample collection and transport, turnaround times, electronic information systems, management of patients with high viral load, and differentiated service delivery for patients with suppressed viral load. It also provides guidance for reviewing documentation, verifying data consistency across multiple sources, and assessing adherence to recommended clinical and monitoring procedures. The tool is intended to strengthen routine quality assurance, improve the reliability of viral load monitoring data, and support more effective HIV programme management and patient care.
