This web annex provides a standardized clinical facility assessment tool to support implementation of the WHO module for assessing and strengthening the quality of viral load testing data within HIV programmes and patient monitoring systems. Designed for use during facility assessments, it enables systematic evaluation of service delivery, laboratory practices, monitoring and evaluation systems, and data quality processes related to HIV viral load testing. The tool includes structured questionnaires, scoring criteria and guidance for assessors to document facility readiness, identify gaps and monitor improvements over time. The annex supports routine verification of the completeness, consistency and accuracy of viral load testing data across multiple data sources, including patient records, laboratory documentation and electronic information systems. It also includes procedures for cross-validation of key data elements, assessment of specimen collection, transport and laboratory processes, evaluation of point-of-care viral load testing where applicable, and documentation of corrective actions. The standardized scoring framework and facility summary report are intended to guide quality improvement activities, strengthen patient monitoring systems, and support reliable viral load data for clinical management and HIV programme monitoring.
