This web annex documents the Evidence-to-Decision (EtD) framework used to support the development of the WHO recommendations on mechanical methods for induction of labour. It presents the evidence considered by the Guideline Development Group when assessing the use of mechanical methods, alone or in combination with pharmacological interventions, for women at or beyond term. The annex defines the clinical question, priority maternal and perinatal outcomes, and summarizes the evidence drawn primarily from an updated Cochrane systematic review comparing different mechanical methods with pharmacological agents and combined approaches. The document systematically evaluates the certainty of the evidence and the balance of desirable and undesirable effects, together with considerations relating to values and preferences, resource use, equity, acceptability and feasibility. It includes GRADE summary of judgements and summary of findings tables that underpin the formulation of the recommendations in the parent guideline. Published to ensure transparency in accordance with the WHO handbook for guideline development, this annex provides the methodological rationale and supporting evidence used in the guideline development process without presenting additional recommendations beyond those contained in the main WHO guideline.
