This Web Annex J accompanies the WHO guidelines on risk reduction of cognitive decline and dementia, second edition, and presents the evidence profiles and evidence-to-decision framework supporting recommendations on the management of stroke for dementia risk reduction. The publication compiles systematic reviews and evidence syntheses examining whether pharmacological interventions for adults with a history of stroke or transient ischaemic attack reduce the risk of cognitive decline, mild cognitive impairment and dementia. It evaluates evidence for blood pressure-lowering, antithrombotic and lipid-lowering therapies, together with their effects on cognition, quality of life, functional outcomes and adverse events, and identifies important evidence gaps where available data are limited or uncertain. The document describes the methodologies used to identify, assess and synthesize evidence, including comprehensive literature searches, predefined eligibility criteria, study selection, data extraction, quality appraisal using AMSTAR-II, risk-of-bias assessment and GRADE evaluation of the certainty of evidence. Evidence profiles, narrative summaries, summary-of-findings tables and evidence-to-decision frameworks present the available findings and the judgements informing the guideline development process. Developed in accordance with the WHO Handbook for Guideline Development, this annex provides transparency regarding the evidence base underpinning the WHO guideline recommendations and supports evidence-informed policy-making, programme development and clinical practice aimed at reducing the risk of cognitive decline and dementia through optimal management of stroke.
