This guideline provides technical guidance for adapting the World Health Survey Plus (WHS+) mobile phone survey questionnaire to national contexts while maintaining comparability across countries and over time. Developed as part of the WHS+ mobile phone survey technical package, the publication describes the structure, conventions and modular design of the questionnaire system used to support monitoring of health-related Sustainable Development Goals and WHO programme indicators. It explains the different survey modes used in WHS+ Mobile, including computer-assisted telephone interviewing, interactive voice response and short message service surveys, as well as the target populations associated with individual survey modules. The publication outlines the process for developing country-specific questionnaires through collaboration between ministries of health, national statistical organizations and survey implementation teams. It provides recommendations on adapting questionnaire wording, selecting modules, managing eligibility criteria and maintaining standardized enabling conditions and response codes. Guidance is also provided on multilingual survey implementation, adaptation for random-digit dialling or recontact surveys, and the incorporation of country-specific questions while preserving indicator comparability.
