This expert consultation report summarizes discussions convened by the Alliance for Health Policy and Systems Research in April 2024 to inform a new programme of health policy and systems research aimed at improving vaccine uptake in low- and middle-income countries. The report examines persistent and emerging challenges affecting vaccination uptake, including vaccine hesitancy, misinformation, inequities, political and governance factors, health system constraints, and the influence of global dynamics. It highlights limitations in existing evidence, which has largely focused on individual-level determinants, and emphasizes the need for research that explores interactions between individuals, communities, health systems and broader political, social and economic contexts. Drawing on the contributions of international experts and policy-makers, the report identifies and refines four priority thematic areas for future research: community and local social determinants of health, health system contexts, national political governance, and global forces influencing immunization. Cross-cutting priorities include digital transformation, health workforce capacity, data and learning systems, community knowledge systems, emergency settings and systemic risks affecting immunization programmes. The consultation informed the development of a call for research proposals designed to generate policy-relevant evidence and support context-specific strategies to improve vaccine uptake, including among zero-dose children and across the life course.
