This web annex provides user guidance for the viral load data quality monitoring tally sheet developed as part of the WHO module for assessing and strengthening the quality of viral load testing data within HIV programmes and patient monitoring systems. It describes the purpose, structure and operation of the semiautomated Excel-based tool and outlines procedures for recording patient-level information from primary and secondary data sources. The annex also explains the application of lot quality assurance sampling (LQAS) to determine sample size, decision rules and assessment thresholds for routine monitoring of viral load data quality. The publication provides step-by-step instructions for completing the data entry and results summary worksheets, comparing selected data elements across clinical and laboratory information sources, and interpreting automated assessments of data consistency. It describes how consistency is evaluated for key variables, including antiretroviral therapy regimen, viral load test dates and results, and patient follow-up information, using predefined decision rules. The annex supports standardized implementation of routine data quality monitoring activities, helping identify discrepancies between data sources and strengthen the reliability of viral load monitoring within HIV patient monitoring systems.
