This web annex accompanies the WHO Guidelines for the care and treatment of persons diagnosed with chronic hepatitis C virus infection and documents the Evidence to Decision process for the PICO question addressing whether all persons diagnosed with chronic hepatitis C virus infection should receive direct-acting antiviral (DAA) therapy. Developed in accordance with the WHO guideline development process, it summarizes the evidence reviewed on the benefits and harms of a “treat all” approach, including effects on liver-related and extrahepatic outcomes, hepatitis C virus transmission, equity, acceptability, feasibility, cost–effectiveness and resource implications. The annex presents the rationale supporting the recommendation to offer DAA treatment to all individuals aged 12 years or older diagnosed with chronic HCV infection, with the exception of pregnant women. It synthesizes findings from systematic reviews, modelling studies and stakeholder surveys, assesses the certainty of the evidence using the GRADE framework, and outlines implementation considerations, research gaps and the overall strength of the recommendation. Published to promote transparency in WHO guideline development, the document provides the detailed evidence and decision-making framework underpinning the corresponding recommendation in the parent guideline.
