This article summarizes the discussions, conclusions and recommendations of the Strategic Advisory Group of Experts on Immunization (SAGE) following its meeting in March 2025. It reviews progress and emerging challenges affecting global immunization, including reductions in international funding, persistent inequities in vaccine access, the recovery of routine immunization following the COVID-19 pandemic, and implementation of the Immunization Agenda 2030. The document examines WHO’s strategic priorities, advances in vaccine research and development, regional manufacturing capacity, and the introduction and optimization of vaccines against diseases such as tuberculosis, malaria and human papillomavirus. It also addresses the importance of surveillance, laboratory networks, digital technologies and strategies to counter vaccine misinformation and hesitancy. The report further presents updates from Gavi and the WHO regions, highlighting progress and challenges in strengthening immunization systems, reducing the number of zero-dose children, responding to humanitarian emergencies, and advancing disease elimination initiatives. SAGE emphasizes the need to sustain political commitment, adequate financing and WHO’s technical leadership to preserve recent gains, strengthen equitable immunization programmes and accelerate progress towards global vaccination goals.
