dc.descriptionThis web annex to the WHO guidelines for the treatment of patients with cystic echinococcosis presents the evidence-to-decision (EtD) frameworks used to support formulation of recommendations for the management of uncomplicated hepatic and pulmonary cystic echinococcosis (CE). The document summarizes the decision-making process applied by the WHO Guideline Development Group for each population, intervention, comparator and outcome (PICO) question addressed in the guideline. The EtD frameworks incorporate multiple criteria, including desirable and undesirable effects, certainty of evidence, patient values and preferences, balance of effects, resource implications, cost effectiveness, equity, acceptability and feasibility. The annex provides structured summary judgements for eleven PICO questions related to the treatment of different cyst stages and sizes using albendazole, PAIR, surgery, standard catheterization, modified catheterization techniques and praziquantel-based combinations. It reflects consideration of varying levels of health system capacity, clinical expertise and resource availability across treatment settings. The document complements the evidence synthesis presented in Web Annex A and contributes to transparency in guideline development by documenting how available evidence, expert opinion and contextual factors informed WHO treatment recommendations for cystic echinococcosis.
