This operational guidance provides a structured framework to support countries in sustaining essential services for HIV, viral hepatitis and sexually transmitted infections during periods of reduced external funding and health system disruption. Intended for governments, health programmes, civil society organizations, donors and technical partners, the document outlines practical approaches for assessing risks, setting priorities and adapting services while preserving continuity of care and protecting public health gains. It responds to the effects of declining official development assistance, including disruptions to service delivery, workforce capacity, supply chains, financing and health information systems. The guidance presents a stepwise priority-setting framework grounded in ethical, procedural and evidence-informed principles. It describes governance and planning requirements, methods for assessing and appraising services and approaches for categorizing interventions according to priority under constrained resources. The publication also addresses cross-cutting system considerations such as primary health care integration, workforce resilience, financing transitions, monitoring systems and community engagement.
