This supplement updates the 2013 WHO consolidated guidelines on the use of antiretroviral drugs for treating and preventing HIV infection by introducing new and revised recommendations on post-exposure prophylaxis (PEP) for HIV and the use of co-trimoxazole prophylaxis for HIV-related infections among adults, adolescents and children. Developed using the WHO guideline development process and informed by systematic reviews and GRADE methodology, the publication presents evidence-based recommendations to support a public health approach that emphasizes simplified, feasible and effective interventions across diverse settings. The guidance is intended primarily for policy-makers, programme managers and health-care providers, particularly in low- and middle-income countries. The document provides updated recommendations on eligibility for HIV PEP, preferred antiretroviral regimens for adults and children, prescribing practices, adherence support, follow-up and implementation considerations, without distinguishing between occupational and non-occupational exposure. It also revises guidance on the use of co-trimoxazole prophylaxis, expanding recommendations on initiation, continuation and discontinuation for adults, pregnant women, adolescents, children and HIV-exposed infants, while considering the prevention of opportunistic infections, severe bacterial infections and malaria. The supplement highlights implementation issues, research gaps, monitoring and evaluation, and supports the integration of these interventions within comprehensive HIV prevention, treatment and care programmes.
