This report examines the environmental impacts of clinical trials and identifies key considerations for developing climate change mitigation and adaptation strategies in clinical research. It brings together work on climate change, health and ethics and ethics oversight, in support of the implementation of the WHO Clinical Trials Resolution (WHA 75.8). Covering the full lifecycle of clinical trials, including pre-trial, in-trial and post-trial activities, the report outlines opportunities and challenges for reducing greenhouse gas emissions and other environmental harms while maintaining trial integrity. It also addresses climate resilience, highlighting vulnerabilities of trial systems to climate-related disruptions and the need for adaptive strategies. Using a comprehensive approach that places environmental and social justice at the centre of trial design and implementation, the report identifies key roles and responsibilities for policy- and decision-makers, funders, regulators and researchers. It places particular emphasis on the needs and capacities of low- and middle-income countries (LMICs). The report is intended to support the integration of sustainability considerations into clinical trial practice and to contribute to the development of low-carbon, resilient health research systems.
