This web annex accompanies the WHO Consolidated guidelines on person-centred HIV strategic information: strengthening routine data for impact by providing a standardized ART cohort report package and practical guidance for analysing treatment outcomes among people receiving antiretroviral therapy. Intended as an operational reporting resource for health facilities and national HIV programmes, the annex comprises standardized cohort report templates, reporting forms, calculation methods, completion instructions and worked examples for monitoring retention in care, treatment status, viral load testing and viral suppression at defined follow-up intervals. It also includes guidance for long-term cohort analysis, monitoring pregnant and breastfeeding women, and assessing differentiated service delivery (DSD) indicators, including programme coverage, retention and viral suppression among individuals enrolled in DSD ART models. By promoting standardized cohort reporting and consistent programme evaluation, the annex supports implementation of the recommendations contained in the WHO guideline.
