This milestone report reviews global progress during the five years following WHO’s 2018 call to eliminate industrially produced trans-fatty acids (TFA) from the food supply. It assesses advances in policy development, implementation and enforcement, and examines their implications for public health. The report describes the burden of TFA consumption, a major dietary risk factor for coronary heart disease, and summarizes actions undertaken by countries, regional organizations, industry and international partners in support of the WHO REPLACE initiative and the global target of TFA elimination. It also presents regional progress, highlights successful policy approaches, and documents developments in monitoring, validation and industry reformulation efforts. The report finds that substantial progress has been achieved, with WHO-recommended best-practice TFA policies in effect in an increasing number of countries and protecting a growing share of the world’s population. It identifies remaining gaps in policy coverage, implementation capacity and enforcement, particularly in regions with a high residual burden of TFA-related disease. Drawing on lessons learned from country experiences, the publication outlines challenges related to laboratory monitoring, industry compliance and replacement of TFA with healthier fats and oils. It proposes a roadmap for achieving sustained global elimination, including expanded adoption and enforcement of best-practice policies, strengthened regional action and continued collaboration among governments, industry and public health partners.
