This web annex provides standardized summary tables for compiling and synthesizing information collected through the WHO COVID-19 vaccine post-introduction evaluation (cPIE) questionnaires. Designed for use alongside the cPIE guidance and questionnaire package, the spreadsheet facilitates the organization of findings from interviews, observations and document reviews conducted at national, subnational and health facility levels. By providing structured templates for summarizing responses, it supports consistent analysis, comparison of findings across programme components and preparation of evaluation reports while allowing adaptation to country-specific contexts and data collection approaches. The workbook contains dedicated summary tables for national, provincial (subnational) and health facility questionnaires, together with templates for vaccine storage and vaccination session observations, interviews with health workers and other priority population groups, stakeholder consultations, and disaggregated data reporting. Covering the principal operational areas assessed during the cPIE, the annex helps users consolidate evidence, identify strengths and implementation gaps, and organize key findings before developing recommendations. It serves as a practical tool to promote harmonized documentation and interpretation of evaluation results and to support programme improvement and future COVID-19 vaccination planning.
