This module of the World Health Organization (WHO) Toolkit for the analysis and use of facility-based data on noncommunicable diseases provides practical guidance on the collection, analysis, interpretation and use of routine health facility data for monitoring noncommunicable disease (NCD) services in primary care settings. The publication supports programme managers, health facility managers, surveillance officers and health workers in strengthening facility-based monitoring systems for cardiovascular diseases, diabetes, chronic respiratory diseases and selected cancer-related services. It outlines approaches for capturing patient and facility data, assessing data quality, defining standardized indicators, and using data to improve service delivery and programme performance. The guide describes core indicators related to service availability, screening, diagnosis, treatment, referral and disease control, aligned with WHO PEN and WHO facility-based monitoring guidance.
