This Web Annex B supports the second edition of the WHO guidelines on risk reduction of cognitive decline and dementia by presenting the evidence profiles and evidence-to-decision framework for social activity interventions. It evaluates whether interventions that promote social participation or prevent social isolation among adults aged 40 years and older with normal cognition or mild cognitive impairment reduce the risk of cognitive decline or dementia. The annex describes the systematic review methodology, including PICO questions, eligibility criteria, literature searches, evidence synthesis, quality assessment using AMSTAR 2, and certainty of evidence assessment following the GRADE approach. The annex summarizes evidence from systematic reviews, randomized controlled trials and observational studies on the effects of social activity interventions on dementia risk, cognitive function and selected secondary outcomes. It presents GRADE evidence profiles, evidence-to-decision tables and summary judgements, highlighting the certainty of the available evidence and identifying important research gaps. Developed in accordance with the WHO Handbook for Guideline Development, this annex provides the transparent evidence base underpinning the guideline development process and supports interpretation of the corresponding WHO recommendation on social activity.
