This guideline provides evidence-based recommendations for intensified tuberculosis case-finding and isoniazid preventive therapy (IPT) for people living with HIV in resource-constrained settings. Developed according to WHO guideline development procedures, it updates the 1998 WHO/UNAIDS policy by incorporating new scientific evidence, systematic literature reviews and GRADE assessments. The document addresses the high burden of tuberculosis among people living with HIV and promotes the integration of intensified case-finding and IPT as complementary components of comprehensive HIV prevention, treatment and care. It provides guidance for policy-makers, national HIV and tuberculosis programme managers, health-care providers and other stakeholders responsible for implementing collaborative TB/HIV activities. The guideline presents recommendations for tuberculosis symptom-based screening algorithms, the use, duration and implementation of IPT in adults, adolescents, children and special populations, including pregnant women and people receiving antiretroviral therapy. It also discusses the role of tuberculin skin testing and interferon-gamma release assays, implementation considerations, monitoring and evaluation, cost-effectiveness, and operational challenges. The publication identifies research priorities, outlines the guideline development methodology, and emphasizes adaptation of the recommendations to national epidemiological and health system contexts to strengthen tuberculosis prevention among people living with HIV.
