This companion document to the WHO guideline: recommendations on digital interventions for health system strengthening identifies priority research needs to strengthen the evidence base for digital health interventions. It synthesizes cross-cutting evidence gaps identified during guideline development, highlighting limitations in the available evidence on effectiveness, resource use, cost-effectiveness, gender, equity and rights, and implementation. The document outlines overarching research priorities, provides considerations for the design of future evaluations, and emphasizes the need for rigorous methods, common evaluation metrics, and implementation research to support the scale-up of digital health interventions across diverse settings. The publication also presents evidence maps summarizing the certainty and extent of available evidence for the interventions assessed in the guideline and proposes intervention-specific research questions covering effectiveness, acceptability, feasibility, resource use, and equity. These priorities address interventions including birth and death notification, stock notification and commodity management, telemedicine, targeted client communication, decision support, digital tracking and digital learning.
